Kathy, if you have the meeting request form enabled on your platform that is what I would suggest using. Or if you create a reg form with the questions you listed here would work perfectly as well. I don't have a form, but would add questions like Reg Deadline, Are guests allowed, fees, agenda etc.

I am looking for help in building out a questionnaire to share with a meeting requestor/stakeholder to get the "backend" questions answered. Things like, who do you want the "from" to be? Do want non-responder emails? how often? Do you want a bounce back report? etc. all the things we need to know to build out the backend of the reg site. Does anyone have one they can share? I had one at a previous job but can't find the file anymore :-(
