To confirm, are you talking about the "Answer Details by Question" report the Premium Surveys tool? If you go to Data > Change Report Data > Fields, you will see that you are able to add additional fields to this report like custom contact fields and some additional respondent fields like completed date (wondering if that would be the same as the submitted date field that you mentioned.) What kind of field is your "lead planner role" created as in Cvent? If it is something other than a standard contact field, custom contact field, response field or respondent field listed out on the available fields tab, which is all that is available to be pulled into this particular report, I would recommend submitting this as a product feedback within Participate > Product Ideas.
Also is there a reason that it is set up as a standalone survey vs. actually being linked to the event in Cvent? (Ex: Enabling the surveys and assessments feature within your event and then setting it up as linked to that event.) If you link it to the event, you will be able to run the reports on it within the given event in Reporting > Reports and there's the Survey Answer Details by Attendee report and you are able to add more fields to these report than you can within the standalone survey (ex: you can add in custom event fields, event general information, more registration information, etc.)

Hello! I currently run report template 'Answer Details by Question from Survey Reports in the Surveys section of the tool. This report provides the name of the Survey which is the name of the event, Response Code, Response Status, Last Modified Date, Chapter (?), Question, Answer and File Link.
I would like to build a survey report that includes additional fields such as submitted date, submitted by and lead planner role.
I attempted on my own and was unable to pull in additional fields / data points. I didn't see a way to build from scratch on the events platform. Anything there is used to pull event surveys the planner sends to event attendees.
Any direction is appreciated!
