I have to think cvent uses one. Our first registration was built by cvent. They must have used a questionaire of somesort.
I went through old emails and found one from 2016 titled Event Builder Form. Once I filled out the form we had several sessions back and forth to discuss the build. Looking at it now, I hope there is something better. :)
