Great question! In most cases, I'd suggest using a multi-select question populated with options you know you'll be able to provide if someone asks for them. An example might be:

___ Wheelchair access
___ Lactation room
___ Gender neutral bathroom

That free-entry "Other" option gives people a chance to request something you hadn't thought of, or add more detail.
Depending on the size and audience of your event, it can be worth getting very specific. Here are a few example questions from a conference on assistive technology where many attendees have disabilities:

I wanted to briefly introduce myself and then let you know about an exciting new Topic we just added.
I am Cvent's Senior Product Manager-Accessibility, I work to make sure that our products are usable and enjoyed by your attendees who may have disabilities.
I am happy to announce that we have added a new Accessibility Topic to the forum. Feel free to ask questions, make comments, offer suggestions.
We are working on adding accessibility-specific information for Knowledge Base, things like image alt text, color contrast, proper heading structure. I would like to hear your suggestions on what info to offer first!
