If you are using the Registrant Details click the pencil to edit the report before running it. Click on the Fields tab and here is where you can choose which fields you want to be added to the report when you run it.
There is a whole section for Custom Contact Fields and by default non are checked. Go through all the sections and check all the items you want on the report and uncheck all the fields you don't want on the report and click Run.
Once the report is shows the correct information you want, I suggest to Save it (under Actions) and it will be saved under the Saved Reports tab. Note: make sure the My Reports dropdown is expanded to see your report. Then you can run your report whenever you need it with the saved information.
To your initial question, why can you see the custom fields, it may depend on which report you are running.
