Do mean like an intake form to get the event details to build the site? If yes, PM me. I have a form we used to use and I might still have a form Cvent provided us when we used their pro services.
We currently have our Event Build Request form incorporated into our Meeting Request Form.

I've used the Event Builder team in the past and they provided a document of all the info they needed. Unfortunately, I don't have access to that file anymore and when I search on Cvent Community it's not shared there.
