Yes, you need to select it to be included. Or you can run a Question Summary report.

Yes; I think it's easiest to just run a "Question Summary" (drop down under the "Reports" section. But you can also select it as a field (drag from the full question list into the reported list) if you are creating a custom report or modifying an existing report.
