I'd set up as 3 separate admission items. This will allow you to associate questions to a particular option. For example, you can ask to enter the team name as one question, person 1's name as the next question, person 2's as the next question, etc. You can also ask if rental clubs are needed and, if so, left-handed or right-handed for each person.
